Advance Care Planning & Communication Skills Workshop
Do you have conversations with people about their care or the care of a loved one? Do you support people in thinking about their care as they become less well? If the answers are yes, then this session is for you!
Topics covered will include:
- Having those conversations with patients and those important to them about their wishes for future care/support – including by telephone and/or video call
- Picking up on cues and supporting people to talk openly about things that may be concerning them
- Recording and sharing people’s wishes for future care
