Declaration
I confirm that I am taking part in the Skydive Challenge for The Kirkwood at my own risk.
I am medically fit and accept that I am responsible for providing my emergency contact details, any medical conditions, allergies, drug allergies or current medication to the event organisers before or on the day of the event.
I accept all conditions of entry and acknowledge that the organisers shall not be liable for death, personal injury, loss or damage as a consequence of my participation in the event, except with regard to personal injury which is caused by the organiser’s negligence. I understand that The Kirkwood reserve the right to alter arrangements and conditions (such as the event venue, date and start time) should circumstances require.
I can confirm that the information I have provided is correct to the best of my knowledge..
